Adult LD Speech and Language Therapist

• The Community Learning Disabilities team are an integrated health and social services team comprising various LD Nurses and AHP disciplines.  They have 2 x Assistant Practitioners who are a shared resource amongst the AHPs.  There is a transition team as part of the wider LD team who work with young adults moving from paediatric services into adult services.  They have a small Intensive Support Team integrated within the wider LD team. 
• Work exclusively with adults with a confirmed learning disability.  Some have additional needs relating to ASD / mental health. 
• Work with people from mild LD to PMLD.  This includes people who challenge services and/or have forensic background / intensive support needs. 
• The locum will work with three ALD SLTs.  Two of which are part time at band 6.  There is a clinical lead for SLT team working     full time at band 7.   
• Caseload will comprise Communication and Dysphagia cases.  This will include some complex case work / complex MDT work.  All health members are sometimes required to support eligibility assessments for adults referred to the team where there is no previous involvement from health LD services and where there is not a clear diagnosis of LD.  This is a small and occasional part of our role but is there none-the-less. 
• Good links with Community Dietetics and both the community Adult Acquired SLTs and the Adult Acute SLTs
• Electronic notes. The system is a social services system – training will be given.
• The team are office based (all in one office) but assessment / intervention is carried out in the person’s home / day centre / college / community.  We are not a clinic so do not have people come to us. 
• The AHPs have weekly huddles.  ALD SLT team have team meetings every 6 weeks, as well as journal clubs, case note audit meetings and videofluoroscopy workshops spaced out throughout the year.  Health clinical leads meet fortnightly to discuss referrals and we attend monthly Clinical Governance meetings. 
• The team hot desk (there are always plenty of available desks) and we support hybrid working if this supports productivity.
