SEND Preparing for Adulthood Casework Manager

Hybrid role - 1 day in office required 

£350 - £400 per day (Depending on experience) 

Temporary Placement 

 

Main Purpose of the Job

  • To manage the Education Health and Care planning and review function for young people in academic year 9 and above, including providing management supervision to a team of EHC coordinators to deliver Education Health and Care plans and all statutory processes in line with statutory frameworks and legislation.
  • To ensure that young people with SEND are participating in education, employment and training.
  • To ensure effective transitions for young people with EHC Plans, securing provision into post 16 and adult health and care provision.
  • To ensure that preparing for adulthood outcomes in EHC Plans are recorded, monitored and reported effectively.
  • To plan and commission post 16 SEND placements for children, both inside and where appropriate outside of ..., ensuring cost effective use of the Council’s resources.
  • To undertake complex case work as required and work with young people (academic year 9 and above) and their families to resolve difficulties to prevent escalation or tribunal challenges resulting in unnecessary cost and conflict.
  • To respond to complaints and enquiries relating to children and young people in school year 9 and above.
  • To monitor spend of the high needs budget relating to children and young people in school year 9 and above.
  • To provide updated information for the Local Offer including the preparing for adulthood offer available to children and young people.
  • To act as Preparing for Adulthood champion within ..., working to secure employment opportunities including supported internships, supported employment and experiences of the workplace for young people with SEND.
  • To engage parents/carers in informal mediation wherever
  • possible to prevent escalation or tribunal challenges and avoid unnecessary costs to the Local Authority
  • To lead on a series of training opportunities for parent/carers and professionals in relation to transition reviews from school year 9 and above
  • To work with colleagues in adult and community services to increase the number of young people eligible for adult services engaging in paid employment.

 

Key Responsibilities

  • To work in partnership with health, care services and a wide range of specialist organisations such as youth justice and parents and young people to secure effective provision and local packages of support for young people with complex needs.
  • To lead and manage the planning and placements for children and young people in year 9 and above who require specialist residential or out of Southend provision.
  • To monitor provision and outcomes for young people in year 9 and above placed in out of Southend provision and attend annual reviews and meetings, brokering changes as required.
  • To present appropriate cases at the Multi Agency Panel.
  • To secure appropriate provision in mainstream schools or otherwise for pupils in Year 9 and above, brokering and negotiating resources as required in a context of good evidence based practice.
  • To contribute to service planning (including arranging SEN team training days) and attendance at service leadership meetings.
  • To set outcomes for the Preparing for Adulthood EHC coordinator team, undertake evaluation of the team’s performance against those outcomes securing feedback from parents, carers and young people to inform future service planning.
  • To support and represent the service at the Area SEND Inspection.
  • To provide advice to schools, post 16 providers and other services about SEN processes and provision as required.
  • To lead on the effective transition to adulthood particularly for those young people who are likely to need adult’s social care services.
  • To work with children’s and adult services and health to ensure that the transitions protocol is updated to ensure positive transitions for young people.
  • To manage the planning and data analysis for those young people who are likely to need specialist support in preparing for adulthood to inform the Joint Strategic Needs Analysis and the SEND strategy.
  • To lead on the effective planning for Young Offenders with SEND
  • To develop positive relationships with all stakeholders, schools, colleges and other providers and with parents, carers and young people ensuring a person centred planning and integrated approach.
  • To undertake evaluation of the team’s service securing feedback from parents, carers and young people to inform future service planning.
  • To maintain information about post 16 and residential provision for young people with SEND and ensure that the relevant information for the SEND Local Offer is up to date.
  • To contribute to service planning and service management meetings.
  • To ensure that the Council’s policies and procedures are implemented including Equalities, Safeguarding and data protection.
  • To ensure the requirements of the SEND Code of Practice are implemented.
  • To deliver training to staff on schools, colleges, parents, young people and other services and undertake presentations as required.
  • To manage the performance of staff with appraisals and regular supervision and contribute to the induction of new staff.
  • To represent the Local Authority at SEND Tribunal hearings, preparing reports and papers as required.
  • To chair multi agency meetings and annual reviews.
  • To deputise for the SEND Service Manager and the Head of SEND as and when required.
