Job Summary:
- Inform and educate patients, carers and members of the public about the safe use of medicines.
- Attend consultant-led wards rounds and provide patient focussed advice and information in order to ensure safe and effective use of medication.
- Monitor drug expenditure within specialty, produce budget statements and drug usage reports, interpret and provide analysis and recommendations.
- Clinically screen and final check prescriptions (including oral, injectable and clinical trial drugs) according to Trust protocols and locally agreed standards and guidelines.
- To fully participate in the medicines management of patients by being involved in drug history taking, monitoring of inpatient therapy and preparing patients for discharge with a view to safety in a domiciliary environment.
- Provide clinical support to the aseptic preparation unit, working closely with quality assurance and technical support to ensure that the aseptic preparation unit functions in accordance with national requirements.
- Be proactive in risk management, ensuring that all clinical incidents are reported and appropriate action taken in conjunction with the Clinical Lead Pharmacist and/or the specialist risk management pharmacist.
- Develop and implement new protocols and procedures for area of work and assist in development of pharmaceutical aspects of policies for this area.
- Maintain the required records for the Cancer Drugs Fund and liaise with Finance Department and fund holder as necessary. This includes being familiar with the use of Blueteq for high-cost drug reconciliation and invoicing.
- Be responsible for the management or supervision of junior pharmacists or technical staff.
Essential Candidate Criteria:
- Registered pharmacist with GPhC
- Master’s Degree (4 Years) plus 1 year pre-registration training
- Previous hospital experience inclusive of clinical pharmacy practice
- Must have experience and evidence of delivering a critical care pharmacy service.
- Right to work in the UK
